He is survived by his wife, Karlie, 3 children and his sister. A memorial gathering will be held from 4-5pm on Wednesday, May 9, 2012 at Beach Funeral Homes-West, 4999 Wickham Road, Melbourne/Suntree with a service to begin at 5pm. In lieu of flowers, please consider a donation in Jack's name to Vitas Hospice in Melbourne, FL or MD Anderson Cancer Center in Orlando, FL
