Chad Jeffery Hays
June 18th, 1973 - February 5th, 2025
It is with profound sadness that we share the sudden passing of our beloved son Chad Jeffery Hays, 51 of Phoenix Arizona on February 5, 2025. Chad was born on June 18, 1973, in Decatur Illinois to Paul and Karen Hays. Chad was baptized and attended Lutheran schools.
He possessed an exuberant passion and zest for life. He always placed his family and friends first. He loved the arts, laughter, traveling, dancing, growing orchids, and learning all that he could. He volunteered as a Ranger at the Burning Man Festival and attended the festival for over twenty years.
Chad earned a MD, JD, MPH, and his MBA from the University of Illinois College of Medicine and College of Law and UCLA. Chad was a Medical Director at CVS Health and held numerous Primary Care Physician positions.
He was a man of immense humor and wisdom. He is survived by his father Paul Hays, mother Karen Hays, sisters Michelle Hays and Jennifer Pope, fiancé Cerrie Gleason, numerous nephews, nieces, aunts, uncles, cousins and countless friends, and special fur babies Nori and Rolo.
Cremation and several Celebrations of Life will take place in the near future. In lieu of flowers, please donate in Chad's honor to St. Jude's Children Research Hospital.
