Michael Kozak, 72, passed away Monday, March 3, 2025. He was born on June 29, 1952 in Chicago, IL to the late Maria Kucharz and John Kozak.
Those left to cherish his memory includes his brother, Joseph (Deborah) Kozak; niece, Deana Kozak; nephew, Joseph M. Kozak; great niece Addison Kozak.
In addition to his parents, Mr. Kozak is preceded in death by his sister, Stephanie Kozak.
